Civil court forms and fees 


In civil cases, standard forms are used by six New South Wales courts and tribunals. The jurisdictions that use them are the:

  • Local Court
  • District Court
  • Supreme Court
  • Land and Environment Court 
  • Industrial Relations Commission
  • Dust Diseases Tribunal

The standard forms are located on the Uniform Civil Procedure Rules (UCPR) website.

It has more than 100 standard civil forms, which have been approved for use by the Uniform Rules Committee. The forms include:

  • Affidavits
  • Examination orders
  • Notices of motion
  • Notices of appeal
  • Garnishee orders
  • Statements of claim

The fees for filing or lodging forms are paid to the individual court concerned.
